В предложении употреблен артикль an так как речь идет, о некой старой женщине. Женщина (lady) - это существительное, и в предложении употребоялется превый раз поэтому мы ставим неопределеный артикль. Когда будем употрблять это слово во -второй или третий раз, то будем писать уже определенный артикль the.Перед каждый существительный должен стоять артикль, так как это исчисляемое существительное и в единственном числе ставим неопределенный артикль a(an). И в основном в конструкциях такого плана как  there is a  и there was a обычно идет неопределенный артикль.
